Claiming under your Personal Choice policy couldn't be easier. Just follow these simple steps:

GP referral

Your GP refers you to a specialist for private medical treatment or investigations.

Call Us
|

Before you incur costs as a private patient, call our team of Personal Advisors on 0870 609 3675 (8am to 7pm Monday to Friday).

Our Personal Advisor will review your policy with you and help to remind you about your level of cover.

3 |
Complete Claim Form
|

AXA PPP healthcare may have to ask you and your GP or Specialist to complete a Claim Form which we will send to you immediately.

In some cases, AXA PPP healthcare may be able to authorise your claim immediately on the phone.

Return your completed form to us immediately if applicable.

4 |
Authorisation
|

You will be notified by phone or in writing as quickly as possible whether your claim for benefit has been accepted.

We will also remind you of important and relevant policy terms and conditions.

5 |
Undergo treatment and submit bills
|

Once you have undergone eligible treatment, arrange for your bills to be sent to AXA PPP healthcare.

In most cases, AXA PPP healthcare will settle invoices directly with the provider of services.

If you then need further treatment, call AXA PPP healthcare to confirm ongoing cover.
